AccoladeCare's policy on Gabapentin

An AccoladeCare doctor can refill Neurontin/Gabapentin. 

Due to the classification as a controlled substance we are not able to prescribe Neurontin/Gabapentin in the following states: AL,MI, KY, TN, WV and VA. 

Before our doctors can send your prescription to your pharmacy we will need a copy of your photo ID emailed to us at [e-mail] you will also need to complete our gabapentin agreement form sent to you via email.
